All independent licensed providers who render healthcare to Class Members under contract with CSN undergo credentials verification prior to being accepted into the CSN Network, and are recredentialed every two (2) years thereafter.

When individuals provide the healthcare services to CSN Members under employment with an agency contracted with CSN, that agency must ensure that a similar credentials verification process has been completed for its practitioners. The agency must also ensure that the practitioners whom it credentials meet CSN's minimum qualifications in the specific professional category.
